While all members of the Duke community are strongly encouraged to participate in the annual flu vaccine program, annual vaccination or an approved exemption is a condition of employment for employees in Duke Health, which includes the Duke University Health System, the School of Medicine, the School of Nursing, and the Private Diagnostic Clinic.
Annual policy compliance is a condition of employment for all DUHS team members. Duke University Health System (DUHS) requires all healthcare workers to comply with our Healthcare Worker Flu Vaccination policy by either being vaccinated annually against the flu or receiving an approved exemption. Duke University Student Health and Counseling and Psychological Services (CAPS) employees will follow the Duke Health policies related to vaccination requirements.
The 2023 deadline for policy compliance is 10 a.m. Tuesday, November 7, 2023.
Applications for medical or religious exemptions should be submitted before Tuesday, October 24, 2023.
Outside Vaccination Reporting
If you have been vaccinated outside of Duke Employee Occupational Health and not by the Duke peer vaccination program, submit official documentation of flu vaccination to https://duke.is/vxsfu. Official documentation must include your Duke Unique ID, and any of the following:
- Print out from MyChart showing employee name, date and type of vaccine administered.
- Documentation from provider with clinic letterhead/ address or encounter form showing date and type of vaccine administered, DOB.
- Pharmacy printout with employee name, DOB, name of vaccine, date of vaccination and location/ address of pharmacy.
It is important: Please write your Duke Unique ID# on the flu documentation, so that the correct employee gets the correct credit! Add your best contact phone number in case EOHW needs to reach you.
Students can also submit documentation of external vaccination to Student Health via email@example.com?.
