Flu vaccines will be provided for Duke faculty and staff at the following locations, which includes Employee Occupational Health & Wellness. Choose a location that works best for you and walk in (if allowed by that location) or reserve an appointment time using the links provided.

**BEFORE YOU ARRIVE**
Employees who wish to receive the flu vaccine must complete the consent process. They can do so up to 48 hours before receiving their vaccination using their computer or a mobile device. Take the Vaccinatee Survey to complete consent and get your personalized QR code. Employees should bring their personalized QR code (printed or on their personal device) at the time of vaccination.

Electronic Process to Register for Flu Vaccination

Duke has an electronic process for flu vaccination registration. This process replaces the paper forms used in previous years.

  • Bring your smartphone, if you have one, for faster registration.
    • You can bring your smartphone to access the fast pass registration line.
    • Use your phone's camera to scan a QR code on-site or visit an special URL to complete the registration.
    • If you do not have a smartphone, you can use a device provided at the vaccination site. Wait times may be longer with this option.
